Wisdom teeth generate more worry than any other tooth in the mouth, and a good share of that worry turns out to be unnecessary. Some come in cleanly and stay put for life. Others cause trouble years before they cause pain. You can't tell which you have by looking in a mirror, and neither can anyone else.
What settles it is an exam with 3D imaging. We look at where each tooth sits, whether there's room for it, whether a toothbrush can actually reach it, and what the surrounding bone and roots look like. That produces an answer rather than a guess.

Do your wisdom teeth actually need to come out?
Not automatically, and that matters, because removing a tooth that was never going to cause trouble is a procedure nobody needed. A wisdom tooth that's fully erupted, positioned in line with the rest of the arch, and reachable with a toothbrush and floss can often stay right where it is. We'll tell you that when it's true.
The reasons to remove one are specific: recurring pain or infection around the gum, decay in the tooth or in the molar next to it, a tooth that can't be cleaned because of how it sits, pressure against its neighbor, or a cyst forming around a tooth that never emerged. Any of those changes the calculation, and most of them get worse rather than better with time.
Age is part of the picture too. Roots are shorter and bone is more forgiving in the late teens and early twenties, so a removal done then is usually simpler than the same removal done at forty. That's an argument for having them evaluated early, not for having them out reflexively.
What the evaluation involves
A CBCT 3D scan shows each wisdom tooth in three dimensions: its angle, its root shape, how much bone surrounds it, and its relationship to the nerve running through the lower jaw. A flat X-ray can suggest those things. A 3D scan shows them.
We review the scan with you on screen rather than describing it from across the room. You see the same image we do, which makes the recommendation easy to follow and easy to question.
From there you get a plan: which teeth, if any, warrant removal, in what order, what the visit would involve, what comfort options are available, and what it costs before insurance. If the answer is that nothing needs doing right now, that's the plan, and we'll keep an eye on them at your regular visits.
The visit, and what comfort looks like
The area is thoroughly numbed with local anesthetic, and for most people that alone is enough. Nitrous oxide, often called laughing gas, is the common addition for anyone who's simply anxious: it takes effect in a few minutes, wears off just as fast, and you can drive yourself home. Minimal oral sedation is available for a longer appointment or a higher level of anxiety. That one is a single tablet you take at home about an hour before you come in, so you'll need someone to drive you here and back.
How long a removal takes depends on the tooth. One that's fully through and reachable comes out much like any other molar. One that's partly covered by gum or sitting at an angle takes longer, because the tooth may need sectioning or the gum opened to reach it. We'll tell you which yours is before the day, not on it.
Where more than one tooth is coming out, doing them in a single visit usually means one recovery instead of several. Whether that's the right call depends on the teeth and on your schedule, and it's your decision to make with the information in front of you.
Recovery, and the one complication worth knowing about
Expect swelling that peaks around day two or three, soreness for several days, and a return to normal eating over roughly a week. Ice for the first day, then warmth. Soft food, no straws, no smoking, and no vigorous rinsing for the first 24 hours.
The reason for the no-straw rule is dry socket, which happens when the blood clot protecting the socket is dislodged before the site has healed. It isn't dangerous but it's genuinely painful, usually announcing itself three to five days afterward with a deep ache just as the first days' soreness had started to settle. It's treatable, and it's largely preventable by following the first-day instructions properly.
Smoking is the single largest risk factor. If you're going to stop for anything, stop for the week after a removal.

Comfort options are separate again. Nitrous oxide runs 125 to 150 dollars for the appointment. Minimal oral sedation is a single tablet you take about an hour before your appointment, and it runs 400 to 475 dollars however many teeth are being removed. You'll need someone to drive you both ways. Plenty of removals are comfortable with local anesthetic alone and cost nothing extra.
Insurance covers wisdom teeth more reliably than most other treatment, often at 50 to 80 percent, since removal is usually classified as basic or major rather than elective. We'll give you a written estimate with your benefits applied before you commit to anything.
